Payroll Integration

You can now integrate your NY State-mandated DBL policy into your payroll process:

DBL Questionnaire, to be completed by the client or broker.
Debit ACH Form, to be completed by the client.
Officer Exclusion Form, to be completed by the client.
Payroll Integration Form, to be completed by the Payroll Service Provider.
Payroll Service Providers can provide data in one of two ways:
1. Per Payroll Feed - Send a .csv file of the payroll check register every time payroll runs, in this format: Sample Check Register Feed
2. End of Month Feed - Send a .csv file after the last business day of the month. Aggregate all unique employees on payroll each month, by work state, in this format: Sample Monthly Aggregate Feed.
Payroll Integration - What to expect
Broker of Record Change - in case you want TotalBen to broker an existing DBL policy.
M.S.M General Agency is the exclusive General Agent of this Payroll Integration option for DBL.

LLC	Automatically Excluded unless requested
C-Corp	Automatically Included unless file the special form
S-corp	Automatically Included unless file the special form
